The details you need to fill on the 1099-G are:

- Payer's name/Address (The address is on the very up-left corner of your form, Department of Taxation and below);

- box 2 - State or local income tax refunds, credits or offsets - The amount from your 1099-G form;

- box 10 - State - the state you live in;

- box 10b - State identification no. - 0.
